How do I get started?
- Step 1. On our contact us page request forms with your child’s name and birthday on the subject line.
- Step 2. An Intake Form will be emailed to you. Fax, email or mail the completed forms to 650-341-5900 or to email@example.com
- Step 3. Once we receive your completed forms, we will call you to schedule an intake appointment.
Will insurance cover my appointment?
Insurance often covers a variable amount of these appointments. However, the recovery of the costs of the appointments, including the testing and supplements is the responsibility of the patient. Appropriate forms will be given to the family at the conclusion of the appointment for submission to their insurance company. The full fee for the appointment is due at the intake. Payment is by cash, check or credit card.
How long will my child need to be a patient?
Each patient is unique. There is no average amount of time for any given individual. The majority of patients have been receiving care for a minimum of 1-2 years.
How frequently will my child have to return to clinic?
The parent should expect that their child will be seen an average of 4-6 times the first year. Depending on the patient’s progress, they will be returning every 6-8 weeks. These are estimates and the actual number of visits is influenced and guided by a number of factors including: the patient’s age, acuity, and test results.
